Contact form information
If you submit the contact form, the site collects the information you choose to provide, including your name, email address, phone number, contact preferences and any optional background or message. Form submissions are processed by FormSubmit and delivered to Scott by email. FormSubmit may retain submission data for up to 30 days. Do not submit Social Security numbers, account numbers, health records or other sensitive information.
How information is used
Information you voluntarily provide may be used to respond to your request, arrange a conversation and evaluate whether products or services may be available. It is not sold through this website. Additional notices and authorizations may apply before an application or transaction.
